If you are covered under an HMO insurance plan, please contact your insurance carrier to update your Primary Care Provider (PCP) to our clinic before requesting an appointment or completing the New Patient Application.
This step is essential to avoid delays in your care. HMO plans typically require that your designated PCP matches the provider you are seeking services from. If this update is not completed in advance, your appointment may be postponed or denied by your insurance.
To ensure a smooth onboarding process:
- Call your insurance provider and request the PCP change.
- Confirm that the change has been processed.
- Then proceed with submitting your New Patient Application or scheduling your first visit.
